Proposal Forms

A proposal form is used for fact finding purposes and provides Insurers and Underwriters the information they need to make an informed assessment of your risk, which will allow them to determine whether to offer you insurance and at what premium. We have tried to make our proposal forms as short and as easy to complete as possible but if you prefer we are happy to run through the proposal form with you over the telephone.

For ease our forms are editable PDFs so you can complete the forms and email them back to us. If you prefer you can print the proposal form, complete it by hand, scan and email to us or pop it in the post.
